Before we start: 

• I hereby apply for membership of BONDS. If accepted, I agree to be bound by the rules of the club.

• Please note that all sections must be completed in order for this application to be submitted. Any application missing information will be deemed incomplete and not considered.

• All information shall be treated in confidence.

• A clear and recent headshot must be included.

• Applicants must be 21 years of age or older to apply

• Both membership dues and initiation fees are subject to applicable taxes and service fees.

• BONDS's current membership dues and initiation fees are due at the time of sign-up.

• Enterprise starts at $3,299 per year, with an initiation fee of $699

• Power Couple is $1,299 per year, with an initiation fee of $399

• Flex is $899 per year, with an initiation fee of $199

• Side Hustler is $499 per year, with an initiation fee of $99

• A letter of recommendation from a current member is not required but is encouraged.

• Monthly required minimum spend on beverage sales, merchandise and ticket events will apply.

• Auto-billing is required.

• This form will take around ten minutes to complete.
